MI Choice Waiver Program Eligibility

To be eligible for the MI Choice Waiver Program, you must:

  • Be 18 years of age or older with a disability that would otherwise require nursing home care or over age 65 in need of nursing home care
  • Have an individual monthly income of $2,094 or less
  • Live in Livingston, Macomb, Monroe, Oakland, St. Clair or Washtenaw counties
  • Be Medicaid eligible

All persons referred to MI Choice receive a telephone intake interview to determine eligibility for an in-home assessment. If a person is not eligible for the assessment, he or she is assisted through referral to other appropriate community agencies.

If the person is eligible for the assessment, his or her name is added to a waiting list and will be called based on available funding. At that time a registered nurse and social worker visit the person’s home. A more in-depth assessment evaluates the person’s physical and mental health; functional status; informal and formal supports; home environment; financial resources; and third party supports, such as insurance.
